## Desk Nameplate Classic Design 3

This custom made classically styled desk nameplate can have your choice of emblems carved each side of the name. **COMPLETE THE FORM BELOW** - Please input name required. You can have ***ANY*** emblem or logo you require or a rank pin cushion. Using the drop down boxes please indicate your choice for the left and right detail. Upload the image files by clicking the **UPLOAD** button to browse for the image file on your computer and follow the on screen instructions.
